I managed to fill this in at nearly 2am in the middle of the heatwave - I'm nearly dead on my feet but can't sleep because of the heat. The form was, thankfully, easy to fill in and didn't need much brain power to do so. If only every other form was as easy as this. Now to wait.....

Website nearly perfect.
Website was fine except for finding my address. I could find no way to scroll to the bottom of the list so having accidentally clicked on the wrong number I had to enter it manually. But for that glitch I would have given 5 stars.
