First payment worked. Then the scam starts
The first payment worked. Suddenly a monthly fee is billed. My next deposit has been on status processing for 6 weeks now and reports an attempted deposit from a gray listed country. I have also tried to contact them 3 times. Unfortunately, never an answer except the info that my request was forwarded to the specialists and I should be patient. For me, this clearly looks like a criminal scam and fraud or at least embezzlement.
Do not use this in any case!

Unethical charges for monthly maintenance fees

The above link to an image shows the monthly 'account maintenance fees' that US Unlocked uses to drain your deposit balance. No other comparable virtual card / payment provider does this. Not PayPal, not Airwallex. Once you deposit money with US Unlocked, there is no way to transfer it back to your bank account. There is no way to cancel your service. If you spend money by making a purchase, each transaction also incurs a very high 'fee'.
There is no cost to US Unlocked for keeping my money. It's greed and exploitation of customers. Do not use this service.
Weird, but works
The service is a bit overly complicated... and weird , (for example requiring a bank transfer to a Bank in the Netherlands... and days for fund deposits to appear on the account,

I was really hesitant to try this service after reading all the negative reviews, but I gave it a go. It really didn't get off to a good start since I couldn't load the card. Late Sunday night I emailed customer service and early Monday morning I got a reply that my home country wasn't included at the moment. They did however ask if I have another address I another country, which I happen to have. After that they changed my location. Then I tried to load the card again, but messed up since I forgot to send the reference number with the payment (to a Dutch bank). I emailed customer service again and they manually contacted their Dutch partner bank and gave them the correct information. In one day we probably sent 20 emails back and forth. And they did all this only so I could put my €50 into the account.
I only give them 4 stars since they didn't inform me about supported countries from the beginning, but they get 4 well deserved starts for a great customer support, that even helped me quickly and politely when I messed up the transfer myself. And now I finally have the US Amazon Prime Video membership, which was the whole point of this ordeal. I'm glad that I didn't give up.

I have been a user for two years and out of the sudden they stopped answering my requests. On August 16th I have transferred 54 $ to my US unlocked account. The transfer was successfull but they never loaded it onto my card. After I asked two times when I will get the refund they simply didn't answer anymore. Highly fraudulent actions here. Be careful!

As soon as I made my account they asked me for a government-issued ID. The site says that this is required for amounts greater than 500 USD which I get but I haven't loaded a single dime yet and they are asking for it now. This is clearly an identity theft scam, to get your data and then sell it. There's no reason at all to do this. I mean, I did upload my document, with sensitive data covered, that should be enough right? Why would they want this data? Well, my document is clearly legit but I don't want to give my passport number and other data to strangers. Anyways it was automatically rejected.
Lots of BS fees - Predatory
Lots of BS fees. To get funds back that were misrouted to them I am being told I have to pay both a load and unload fee and potentially a wire fee too.

We provided steps on what you need to do before we can submit an inquiry:
1. Send us the proof of payment - which you have already done
2. Login to your US Unlocked account and create a load request so there will be a payment reference # where we can link the payment and we can submit an inquiry - we are still waiting for your update that you have done this step.
We understand that you want to have the funds returned, but we cannot return the funds unless it has been loaded to your US Unlocked account.
Here are a few things to note:
1. Funds must be loaded before a refund can be initiated
2. Refunds are subject to a non-waivable $11.50 Fee.
3. In some cases where we have to issue a wire transfer due to not being able to refund through our payment processor (Money that has been in the account for over 5 months), there will be an additional $25 fee.
4. Load fees are non-refundable.
We look forward to your reply that you have already created a new load request and payment reference number so we can start submitting an inquiry.

The worst customer service in the world!
It is impossible to verify my identity. Tried over 10 times. They have now frozen my account and I can not access my money. Customer service does not respond.
They have finaly responded after this review.

USUnlocked works as claimed
USUnlocked works as claimed. All the negative reviews are either people who don't understand how Prepaid VCCs work or cheapskates who don't understand that businesses need to charge service fees to keep running. They have a lifetime $15 membership fee, load fee $4.59, transaction fee $0.50 and voiding fee $1.00 (PayPal). Not all VCCs will work so you have to generate a new VCC and try it with the merchant. My first VCC didn't work and the second one did it. All in all, I am satisfied with the service. They have a user-friendly transparent dashboard which shows you all the transactions and charges. Funding the account takes 2 to 3 working days. USUnlocked is not a scam, it does work. But of course, the charges and fees may put you off if you expect a low-fee service.
Waste of time
TLDR : Wasted our time and money for a service that didn't work.
We first tried to use the "one time use card" to buy a phone from B&H(which they mentioned was a supported merchant) and it failed. Had to wait for 10 days for it to credit back to our usunlock account. The customer service said we should use the "merchant only" card and so we tried a 2nd time on target(also supported merchant)...
And guess what. It failed too. We decided to ask for a refund.
And it took about 2 weeks until they finally refund it.
We lost about 60usd for something that didnt work. Customer service wasn't professional too but just happy we got our money back.
Pathetic
Just a terrible UX with the worst possible attempt to explain how to use their alleged 'service'. Stay away.

Plus now they have introduced a monthly fee which I never agreed to that eats into my balance. They are being smart so that they don't have to pay back what they owe clients.
