OPEN LETTER
To: Eli Lilly and Company Leadership, Regulatory Bodies, and Healthcare Professionals
Subject: Urgent Safety Risks of Compounded Tirzepatide Mixed with B12
As public concern grows regarding the widespread availability of compounded GLP-1 medications, this letter addresses the immediate dangers of "personalized" tirzepatide products mixed with vitamin B12, as reported by Eli Lilly's recent internal testing.
The Dangers of Blending Tirzepatide with B12
Lilly’s analysis of compounded products marketed as "tirzepatide with B12" (methylcobalamin, hydroxocobalamin, or cyanocobalamin) has revealed significant safety issues:
Formation of Unknown Impurities: Testing uncovered that a chemical reaction between tirzepatide and B12 creates a new chemical entity—a "reaction-related impurity".
Unknown Human Health Impact: Nothing is known about the short- or long-term effects of this new impurity in humans, including its potential toxicity, immune reactions, or impact on efficacy.
Altered Metabolism: The reaction produces a molecule larger than standard tirzepatide, which may alter how the drug is absorbed, distributed, metabolized, or eliminated.
Inconsistent Potency and Contamination: Testing has shown inconsistencies in the amount of active ingredient, with some samples containing as little as 43% of the labeled dose. Furthermore, compounded samples have exhibited bacterial contamination and high endotoxin levels.

Why Compounding Pharmacies Should Stop Producing This Mixture
While compounding pharmacies serve a purpose in personalized medicine, the mass production of tirzepatide with additives has bypassed critical safety standards:
Additives Are Not "Personalized": Most compounders are adding the same additives (B12, glycine, carnitine) to all products, contradicting the definition of personalized care.
Lack of Testing and FDA Approval: Tirzepatide has never been studied in combination with B12. Compounded products are not approved by the FDA for safety, efficacy, or quality.
End of Shortage Rationale: The FDA declared the shortage of tirzepatide over in late 2024, removing the primary rationale for allowing compounded alternatives.
Misleading Marketing: Many compounders and telehealth entities are marketing these untested, potentially harmful combinations as safe alternatives to branded Mounjaro® or Zepbound®, despite evidence of high impurity levels.

Conclusion
The "Tirzepatide plus B12" story is no longer merely a branding dispute; it is a "buyer beware" situation. Testing has demonstrated that this combination creates a "novel, widespread impurity". Therefore, we urgently call on regulators to enforce a recall of all compounded tirzepatide combined with untested additives like B12 to ensure patient safety.

Below is my original 1 star review and updated remarks:Credit card was charged $449. NEVER received meds. I keep getting texts telling me they are "escalating" it, but it is just a waste of time, no one has credited me back, offered to fix the problem. During this time while I was waiting Ely Lilly released a public letter stating the compounded Tirzepatide mixed with b12 is actually dangerous to take. Also, stickers on viles that were intended for insulin, is not very reassuring. (They told me to disregard the expiration and the vile will have a sticker over it) I couldn't confirm this was the case, as it never was shipped to me.
Updated: I reached out to my credit card company who reached out to you, and only at "THAT TIME" did you refund me, after my credit card company had reversed the charges. The "management" team you claim to have reached out to me was a newly hired low knowledge level individual who merely pacified me with telling me the order was at the pharmacy and there was nothing "he" or anyone could do about refunding me once it reached the pharmacy. I asked for him to confirm which pharmacy had received the order and he could NOT confirm this. Nor could he provide a tracking number. Even if it takes 10 business days to receive your product, the product was NEVER shipped to me not even 15 days post ordering. There is clearly a problem with supply. Likely due to the new pressure compounding pharmacies are being scrutinized for by blending with B12.

I would do zero stars if I could!! Scam
I received medication with a different concentration and dosing instructions than what was prescribed which raised safety concerns. When I contacted Orderly Meds, they did not provide a resolution, would not correct the order and was not offer a refund. Customers are not given transparency or choice regarding the compounding pharmacy used. When you order two months and they send you half of what they sent you the last time and give the reasoning as it is more concentrated?? Beware!! Switching companies!!

I'm a patient and everything about the meds has gone smoothly in itself. I would have rated it five stars up until today. So why one star? Something really disturbing happened that honestly made me lose trust in the brand. While at work I get a message from my secretary saying "David" from Orderly Meds is on the line. I tell her I have no idea who that is and to hang up. I obviously know who Orderly Meds is but I value my privacy and I don't want my work knowing personal things like this. To be clear, the only thing I ever shared with Orderly Meds is my personal cell and personal address/email. Anyhow, as you can imagine I'm pretty upset and contact Orderly Meds. The rep seemed concerned but she couldn't provide any clarity on the situation... she was just as confused as I was. She had no idea why someone from Orderly Meds contacted my work and she couldn't tell me if a 'David' even works there. No explanation could be given and it remains a complete mystery. I'm left wondering about the possibilities. I feel there's only two possibilities here and both are pretty disturbing. The first possibility is it truly was Orderly Meds and they went out of their way to find my work info. Why would they think this is acceptable?! The other possible scenario is more disturbing... perhaps it wasn't Orderly Meds which means someone knows I do business with them and is phishing for information. Literally no one knows I use Orderly Meds. My wife doesn't even know... she just knows I'm on a GLP-1 drug (and she's the only one who even knows that). As you can imagine, I'm left feeling pretty disturbed by this.
If Orderly Meds reads this: I really need you to investigate further and provide me with a definitive answer as soon as reasonably possible.

I just wanted to say, orderly has the best customer service. This is my first order with them and everything is moving extremely fast. Even though they told me 8-10 business days, it’s been 2 and I’m already prescribed just waiting for it to be shipped out. I’m absolutely happy with the customer service.
I’ve been getting my tirz through a private doctor and paying more than double of what orderly is charging me for the same dose. I was skeptical because of buying it online.. and switching but I’ve already spoke with a doctor through orderly, and everything has gone so smoothly so far.
I’m so upset I didn’t save money sooner
So happy to be starting my already year long journey with a new company. And I’m so happy they have amazing customer service. They call you back and even text you when you need help.!!
UPDATE:
THIS IS MY FIRST ORDER WITH THEM, I ordered on a Wednesday afternoon around 1pm, I received my prescription the following Thursday via ups around 1:30 pm, THE PACKAGE DOES COME WITH 2 ICE PACKS AND IS WELL WRAPPED!! (So even if you don’t get home til later, the meds stay cold, I didn’t know this and rushed home to put them in the fridge but for anyone wondering..) ALSO in the app it still says my order is processing, the only down side is I did not receive tracking info for my order nor did I know it was being delivered, when you log in , my order still says it’s at the pharmacy..
ALL IN ALL HAPPY, but I’ll keep you guys updated on the potency of the product!

I'm pleased with my experience with OM. A very budget friendly company with good customer service. They would do weekly check-ins to see how things are going and if I had any concerns they would always a follow up to make sure I was doing ok.

Update: Unfortunately it took a Trustpilot negative review to get the attention of management. Brianna was excellent to talk to and completely understood my frustration with lack of communication regarding a refund. The refund was issued and if they were all built like her in terms of Customer Service this would be a different company. Unfortunately, my review still stands and I cant recommend doing business with this company because of the hoops I had to jump through for a refund for services that weren't even rendered.
My experience with OrderlyMeds left a pretty bad taste in my mouth.
The process seems smooth at first, but things change quickly if you end up not qualifying or needing a refund. They are very quick to charge your card, but when it comes time to give the money back, everything suddenly moves at a snail’s pace.
Even after being told a refund would be issued, it turned into a long stretch of waiting, no updates, and follow ups. And if you’re expecting a quick or helpful response by email, don’t hold your breath. Getting a clear answer in a reasonable amount of time felt nearly impossible.
What really rubbed me the wrong way is how backwards the whole thing feels. They are efficient when it comes to taking your money, but noticeably less motivated when it comes to returning it.
At a certain point I stopped wasting my time and just let the credit card company handle it. Honestly, I’m glad I didn’t end up doing business with them.
There are plenty of telehealth companies out there. Based on my experience, I’d recommend looking at other options before giving these folks your money.
