I love this app and the providers really help me get on track with my weight loss

This is the best diet plan I’ve ever been on. The check ins with the Dr and nutritionist helps tremendously. I’ve lost 107 pounds. It’s great.

Have been feeling good and confident, losing most of my pot belly (14 pounds since we started. My doctor Nirav and dietician Rebecca are knowledgeable and encouraging, both have responded to questions even between appointments.

I had a horrible experience with the doctors being overly invasive into multiple medical records unrelated to my weight loss journey and making it as difficult as possible to get zepbound which I’m using for inflammation and insulin sensitivity. Insurance forced me to use form health instead of going through my endocrinologist who wanted to oversee my usage with type 1 diabetes and insurance wouldn’t allow it. Form health has poorly educated doctors who have massive egos and aren’t interested in actually helping patients and avoid doing their jobs. I would ask insurance to go through another option if at all possible. I’m using it for real medical reasons and they did not care nor were willing to work with me Providing just my main medical record. Absolutely horrible service and people working for this org. They dropped me without any warning and have left me scrambling to pay out of pocket because I called them out for asking for medical records unrelated to zepbound. They asked to speak to multiple doctors that I see on the regular and they all approved the medication. Truly a bunch of assholes at form health with no compassion or understanding what so many of us are dealing with who have chronic disabilities such as type 1 diabetes and the constant time spent at doctors. I have never in my life had an org so invasive and horrible to work with.

Exceptional teamwork and helpful guidance in attacking my night binge eating (literally diagnosed ) I immediately was on ozempic w great results but went off because of painful digestive side effects. After several months of plateau, I have kept the weight down to the original -30 I lost. Thru patience, expert support, learning, slowly changing, using all my resources (for which I am most fortunate and grateful) , I hit the -40 lb milestone and gained confidence I will achieve the -60 lb goal in the next severa I have really wanted too succeed as did all Human Resources of Form health. They are obviously skilled, caring and take their “patients/clients”’” goals and unique make up seriously. Highly recommend!

The customer service the friendly physician, and the accessibilities.

I like it. It gives me options to choose my time, and if I need to reschedule due to my extremely crazy schedule, I can do that.

Very supportive. Great people to work with

So far I have not gotten much help at all. Very disappointed with this way of medical care. They always need my medical records from my personal doctors with whom I would rather be working face to face with. I do not trust working with doctors who have no idea who I am. I am not comfortable sharing my medical data over the internet with someone I have not seen in person.

The staff are supportive, knowledgeable and helpful. The app makes it easy to fill prescriptions, get answers and make appts. I’ve lost 60 lbs in a year. Could not have done it without Form.

Form Health is completely amazing! Their communication is top notch! They work around my schedule so well. My doctor is great! My dietitian, Nicole, has helped me tremendously. She has helped me learn how to eat without depriving myself of things I love. I have dropped almost 20 lbs. 10 out 10! Highly recommend!
